NEW CBRF FACILITIES

Matt Talbot is in the process of constructing two innovative, recovery-oriented Community-Based Residential Facilities (CBRFs)
specifically designed from the ground up for long term care clients who are chronically mentally ill. The clients may also have co-occurring physical
or other disabilities. These CBRFs have been conceptualized based on state-of-the-art technologies in terms of both programmatic and service best
practices; and construction design and methods. Our goal is to provide a safe, secure, least-restrictive, serene environment in which clients can
live with dignity; and seek to realize their full human potential. The CBRFs will be licensed by the Wisconsin Department of Health, Division of
Quality Assurance. They will be staffed 24-hours per day, 365 days per year with trained Human Service Technicians, nurses, social workers and
counselors; all working under the direction of a Medical Director. Matt Talbot Recovery Services…dedicated to promoting with skill and compassion
each individual client’s recovery and re-integration into the community.

Client Groups Served: Emotionally Disturbed/Mental Illness

Provides structure, supervision and socialization for individuals with a mental illness. An individualized treatment plan is developed for each client, including: services related to activities of daily living to address barriers to success in independent living; and to assist the client to gain and utilize skills related to personal hygiene, household tasks, transportation utilization, medication adherence, and other day-to-day requirements of living. All services are provided in the least restrictive environment and include: communication and interpersonal skills training, community skills development and enhancement, diagnostic evaluations and specialized assessments, employment-related skill training, medication management, physical health and monitoring, psychoeducation, recovery education and illness management, and substance abuse treatment.
